THE PROMOTION:
This Promotion is sponsored and administered by Health Monitor Network® in conjunction with the 2023 Point of Care Now Annual Summit (the “Event”). By entering the Promotion, you agree to comply with and be bound by the following Official Rules (the “Rules”)
ELIGIBILITY:
The Promotion is open only to legal residents of the United States and the District of Columbia, who are eighteen (18) years of age or older, and is void in Rhode Island, Puerto Rico and where otherwise prohibited by law. Employees, officers and directors of Health Monitor Network and their immediate families are not eligible to participate in the Promotion. All federal, state and local laws, rules and regulations apply. Participation constitutes the participant’s full and unconditional agreement to these Rules and the decisions of Health Monitor Network, which are final and binding in all matters related to the Promotion.
LIMITED TIME ONLY/PROMOTION PERIOD:
For purposes of these Rules, the “Promotion Period” shall be the period beginning on March 22nd, 2023 and ending on March 22nd, 2023.
HOW TO ENTER:
In order to enter the Promotion, you must scan the promotional card at the Event or otherwise complete the promotional card available at the Health Monitor Network booth at the Event. Participants will be required to provide Health Monitor Network with their: (i) first and last names; (ii) mailing address and (iii) email address (collectively, the “Registration Data”) in order to facilitate delivery of the Promotion prize.
You may only submit one (1) entry during the Promotion Period, and any entries submitted by the same individual in excess of the aforementioned limit will be discarded. Health Monitor Network reserves the right to reject any entry that it believes, in its sole discretion, is fraudulent, incomplete or otherwise invalid.
By entering the Promotion, each participant agrees to receive marketing messages from Health Monitor Network in accordance with the Health Monitor Network Privacy Policy (“Privacy Policy”). See the “Privacy Statement” section below for detailed information concerning the Privacy Policy.
PRIZE DRAWING:
Within 1 week after the Event, Health Monitor Network will randomly select one potential (1) Prize winning entry. The potential Prize winner will be notified via e-mail after the winning entry is selected. Entrants should ensure that their Registration Data is accurate and up to date. The potential Prize winner is subject to eligibility verification.
PRIZE DESCRIPTION:
The Prize winner will receive an Apple® Watch Series 8 (with an approximate retail value of up to $500.00 (the “Prize”). In the event of the unavailability of the Apple® Watch Series 8, Health Monitor Network reserves the right to substitute a similar value prize.

ODDS OF WINNING:
The odds of winning the Prize depend on the number of valid entries received during the Promotion Period.
TAX INFORMATION:
Federal, state and local taxes are the responsibility of the Prize winner.
PUBLICATION:
Promotion entry constitutes permission for Health Monitor Network to use each participant’s name and likeness (including pictures) for advertising and promotional purposes without further compensation, except where prohibited by law.
RELEASE:
By participating in the Promotion, each participant agrees to release and hold harmless Health Monitor Network, as well as its legal representatives, affiliates, officers, directors, employees and agents from and against any and all claims or causes of action arising out of participation in the Promotion or receipt of the Prize.
PRIVACY STATEMENT:
The Registration Data collected in connection with the Promotion will be governed by, and used in accordance with the, Privacy Policy.
